Information About the Verification Interview

The application process can take up to 2 hours to complete. 

If you are late, your appointment may have to be rescheduled.  Prior to your application you will be required to watch a 30 minute video giving you an overview of the Ontario Works program.  After the video, a caseworker will meet with you in order to complete your application for assistance.

Ontario Works Applications

The following information is required for you and each of your family members including any children in your care:

  • Birth Verification

  • Social Insurance Numbers

  • Health Cards

  • Immigration Papers (if applicable)

  • Verification of your accommodation expenses.  For example, rent receipts, mortgage papers, utility bills etc.

  • Verification of your income and assets.  For example, pay stubs, RRSP statements etc.

  • Official documents regarding child support and/or alimony and Family Responsibility Office case number (if applicable)

  • All bank account information for the past 30 days showing all transactions on the account and current balances.

  • Vehicle ownership (if applicable)

  • If you have recently been dismissed or left employment, your Record of Employment

  • If you have dependent children attending school, verification of school attendance

  • A void cheque

Temporary Care Assistance Applications

In order to complete your application for temporary care assistance,  the following information must be provided at the time of your verification interview.  The following information is required for each child for which you are applying for temporary care assistance:

  • Birth Verification

  • Social Insurance Number

  • Health Card

  • Immigration Papers (if applicable)

  • Verification of income or assets of the temporary care child.  For example,  support, Canada Pension benefits.

  • Official documents regarding child support and/or alimony and Family Responsibility Office case number(if applicable)

  • All bank account information for the past 30 days showing all transactions on the account and current balances.

  • A void cheque if you would like your payments to be issued direct bank deposit.
